1. Be it ordained by the Town Council of the Town of Millersburg, Elkhart County, Indiana, is ultimately responsible for all systems and forms to be used.

2. Banyon Data Systems & forms will be utilized for the financial management of the Town of Millersburg.

3. We will abide by the form approval requirements as stated in the “Cities and Towns Bulletin" and during audits by the State Board of Accounts.

4. We will notify the State Board of Accounts if desiring to discontinue using this system which has been approved by the State Board of Accounts.

5. If we desire to use any forms which have changed since the date of original approval for Vigo County Public Library in October of 1986, and these forms have not received a written approval from the State Board of Account’s office, we will immediately submit those forms for approval.
